Info Leaving 3rd Dx blank VS putting something not probable Dx

Hi guys i am just thinking a lot of my friends did not always put three dx in PN.
but i am just thinking which is better leaving 3rd dx blank or putting something bit irrelevant

FOR EXAMPLE----> in case of pt c/c feeling of light headed and vision getting dark at time of episode
i put following d/d in PN;

1) orthostatic hypotension
2) vasovagal syncope
3) TIA

during my case closure she asked what is TIA i did mention that is this this this and sometimes you pass out and she is like i did not pass out...

BUT still i put TIA in pt notes. Even though no weakness,numbness etc was there to support diagnosis of TIA..

what you guys think of it..
