Default about patient notes

hello guys,
i had my exam a week ago.It was fine i guess but i have some concerns :

1.i wrote every pn in bullet style and also i followed this pattern:
PMHX : bla bla bla , MEDS: blah blah , PSH: blah....... (u got the idea ).

2.some of my dx/d were not in the order of most likely , is this fatal mistake ?

3. 6 cases were with 3 dx/d and the others with 2 dx/d , what do u think?

4. plz guys any fatal mistakes i have to know about so i won't be surprised ?

this is my 2nd attempt and failed in ICE in te 1st time ( bcz most of my dx/d were w/o supporting )

Most probably u will do fine......if u wrote ur notes well
1)doesn't matter how u write , if u have written it with some logic and understanding u will do fine
2) some of my ddx were not in order , but I did grt in ice
3)I had 3 dds on7 cases , 2 in 3 n 1 in two cases.i did make up a few dds just to fill space , but they were long shots...u need to support ur dds
Nothing is fatal in cs , unless u do sumthing wrong over and over again
