Missing the major diagnosis?
If I miss the number one diagnosis, are they going to fail me?
Like for example a case of Migraine in which I gave tension headache, brain tumor, cluster headache, temporal arteritis, ....etc but did not give Migraine! Does that constitute a complete failure in the exam!

If you get the primary diagnosis right you pass, if you don't get it you can still pass the case if you placed reasonable alternative diagnoses.
In the example you gave, I think you pass the ICE component
