Question ECFMG Form 186 (Certificate of Identification) versus USMLE Step 3 (Certification of Identity)

For Step 3, is the Certification of Identity form the same as the Certification of Identification (186) form submitted to ECFMG?

And is there any other documents to be mailed for Step 3 application?

Arrow Certification of Identity is different from Form 186

No, it's not the same.

The Certification of Identity Form is another form required by the FSMB for your USMLE Step 3 application.

You can download the form from this page
http://www.fsmb.org/pdf/USMLEStep3_C...f_identity.pdf

It has to be notarized by a notary public (no need for med school official) and you then have to mail it to the FSMB.

Just like Form 186, it's valid for 5 years.

Do I need to send other documents?
No. If you are applying online, all you need to send is the Certification of Identity Form

you have to do the application online and then print it and send it to their address and also to send the identification to the same address?

Do we have to print out the online application and send it as well?

Hi

I dun think there is a need to print the online application
Based on fsmb website
Just print out the Certificate of Identification and send it after notarisation.

Am i right or am i wrong?

Where can we get this form signed from outside of the US?
Can I sign it at US embassy in a country that's not my home country?

The certification of identity form asks for the SSN. What if you don't have one? I've checked the USMLE bulletin, and it doesn't say you need one to register and sit for the Step 3.

Any input will be appreciated.

Nope, I think what he said applies only to the SSN. And I confirmed that with a FSMB representative.

Now, you HAVE to choose a State Board because you can only apply to some of them before residency, and the cost is also different according to which board you are applying to (most charge the same, except 2 or 3 states)

Means If I register for step 3 in Connecticut, then I will write Connecticut in State Licensing Authority for which Step 3 is being taken ?

These questions have been answered thousands of times!

in SSN write USMLE ID.

IN state Medical board write any of the 14 states that dont have a residency requirement (Connecticut is one of them).

When you schedule your exam via prometric you will be able to take the exam ANYWHERE IN THE U.S, from FLorida to California, from the state of Washington to the state of Maine and so on
