Hey this is my opinion about Step 3 - but please ask around for more opinions and of course facts are better...

most states in the US allow MDs to take Step 3 before residency, some states ask that you complete at least 6 months of internship (PGY1) before registering for the exam. The exam is registered at the Federation of State Medical Boards not the ECFMG - you have to have a ECFMG certification and M.D./graduation from your medical school in order to apply.

when should you take it? Whenever its optimal for you. To get into residency - it doesnt seem to have a strong significance when it comes to selecting residency candidates for interview or the rank order list for the match - esp not as much as Step 1 and Step 2 CK scores, Letter of recs in the department you are applying and clinical clerkships (esp those which were done in the field which you are applying) - that and your personality and a good clean white teeth will get you a residency in the U.S. Step 3 is more like a formality to get licensed in the state of your residency and/or career practice - you cannot sit for your Board Certification without Step 3 (I believe) and Board Cert happens the following August upon completion of your residency (which will be in 4 years for you if you get selected in the Match next year)

Also, Step 3 is a LOT like Step 2CK in that - they will ask questions from every subject and more management and micromanagement questions (MKSAP and Step 2 notes as well as USMLEWORLD) - and its a 2 day test with a CCS clinical cases in which you have to turn the clock forward and move a patient from the ER to whichever location in the hospital for you to do the subsequent steps in that patient's management based on chief complaint.

but it requires less time for you to study - and the score doesnt really matter - EVEN if you are planning Fellowship (this is what I heard)

Best.
