I'm so confused what did that mean, why did they put up score report date when i didn't even take the exam can you pls tell me what it means?

It means that you did not take the exam.
They put the information under usmle exam history but they will never report it to any authority, if you ask for a USMLE transcript, you won't see any attempt history.
Am 100% sure about this, so don't worry.
