Default Non-Anion Gap Metabolic Acidosis Question

A 45 yom visits for routine exam. PmHx includes DM1 and GERD. His temp 37.0ºC (98.6ºF), pulse 65, respirations 20, and bp 126/62. PE shows no abnormalities. Labs:

pH 7.30

PCO2 35 mm Hg

K+3.3 mEq/L

Na+ 144 mEq/L

Cl- 115 mEq/L

HCO3- 20 mEq/L

BUN 12 mg/dL

Creatinine 1.0 mg/dL

Which of the following causes can lead to this patient’s abnormal blood pH?

A) Diarrhea
B) Vomiting?


Ans: Diarrhea... what differentiated diarrhea from vomiting if they both present with a non-anion gap metabolic acidosis with hypokalemia?
