Default help please

I also have a question about transcript. I lost my step 1 result and want to get it as transcript (as I understand this is my only option) for myself not to give it to anybody else but in form 172 there are options as:''Enter the name and
address for each individual or institution that is to receive a copy of your official
USMLE transcript.''
https://www.ecfmg.org/forms/form172.pdf
I dont want to enter any other name I just want it to be sent to me.

complete Form 172, Request for an Official USMLE Transcript, and return it to ECFMG with your payment. Currently, the fee for requesting official USMLE transcripts is US$70 for each Form 172 you submit.
