Default Do we really need to do a fellowship even if we don't like to ?

Let me get my point straight. The reason why I am asking this question is because, Do we as an IMG really have any advantage on doing fellowship especially in respect with VISA or something or Its just matter of pursuing our goals?

It is a personal choice. If you like to become a specialist because you have an area of interest and/or want to earn more than general practitioners, it is totally up to you whether or not to go for a fellowship. Nothing to do with Visas.

If you want to be just a primary care physician at your own private clinic, it is your choice.

it depends on you personally, if you want to complete fellowship then complete. But fellowship means physician is permitted to practice without direct supervision by other physicians in the sub-specialty
