Default Need advice

Hey guys i hope you are all doing well.
I am an IMG, and i passed step 1 and step 2 ck with 234 and 240 respectively.
I haven't applied to step 2 cs.

Im a 2 years general surgery resident in my country (europe) and i have a lot of clinical experince.

My question is what should be the best next step for me? Should i apply step 2 cs then apply the surgery programs? Or do i need some clinical experince(observership, research programs) in the usa? But how?
Anyone can give me advice? Thank you for attention, and good luck for all of us

In order to apply for the Match as an IMG, you must be ECFMG certified. And I would suggest you be ECFMG certified before your application is released to programs (mid September on each year). Some will argue & say that one needs to be ECFMG certified by the rank order list deadline (mid February) or something like that, which is true but programs won't invite an IMG for an interview if they are not ECFMG certified.

In order to be ECFMG certified you need to have your medical diploma (= diploma delivered at end of med school) verified by ECFMG - it can take a long time - and pass Step 2 CS (you must travel to the US to take it). Once you pass your first Step it is valid for 7 years. So if you took your first Step six year ago you need to hurry up and take Step 2 CS soon, or you will have to retake that Step.

You need LORs from US doctors, which means you need to do clinical rotations in the USA. I think it would be very hard to match without US Clinical Experience (= USCE). I am not saying "impossible" but definitely "very hard". LORs from your home country can be helpful but are not as helpful as US LORs.

The kind of USCE you can get in the US varies depending on your status in your home country. What does it mean ? It means that if you become a fully certified doctor in your home country you will not be able to do rotations at a hospital, and will only be able to shadow there. The only hands-on experience you could get is at a private practice of a doctor, who would be happy to work with you. If you are not a fully certified doctor in your home country you can then (I think) apply for electives or clerkships at US hospitals, which means you can work with medical students and get some hands-on experience at a hospital. You will need to double check everything I just said but it is sure that depending on your status in your home country, the type of USCE you can get in the US differs.

So what should you do ? Make sure you had your medical diploma verified by ECFMG, study & take Step 2CS (in the US). While being in USA get some clinical experience - 3 month is a good amount - and then apply for the MATCH.

Try to get USCE clinical experience in US which is more useful and try to gain LORs which helps the application. You should apply for match after getting step-2 CS score because programs likes to have applicants to complete the requirements

you don't need to be ECFMG certified at the time of application for most programs just as you become certified before MATCH day. HOWEVER you will have a muuuuch higher chance of getting interviews if you have already taken CS at the time of your application.

I wanna share the experience my friend had because I think it will be useful to you. She applied for residency with just step1 and CK results, and her scores were average, lower than yours. She did not get any interviews. This year she applied again, but now she has CS AND STEP3 results. She got a bunch of interviews, and from places like Mayo (minnesota) and Case Western. I should add she has done a clerkship in US and research. It is extremely useful for IMGs to have step3 on top of being ECFMG certified, especially if they need visa. Also, you should have experience in the US. Research is also helpful, especially if you're published! this research can even be from your native country, as long as it is published in a respectable journal.

Also, surgery is very competitive, so your CV needs to be stellar!
