a 2 yr old boy has had a purulent drainage from the right nostril for a week. he is afebrile and has had no symptoms, such as cough. which of the following is the most likely diagnosis?

a. sinusitis
b. nasal polyps
c. a URI
d. a foreign body in the right nostril
e. allergic rhinitis

In asymptomatic children with fixed purulent foul smelling discharge the differential narrows down to foreign objects, thus I would go with (D)

Yes I agree with my fellow's explanation.

But one question though, why foreign body makes to have purulent discharge? Is it because of infection secondary to obstruction by foreign body?

yep. foreign body in the nose blocks the drainage of secretions from paranasal sinuses. accumulated secretions lead to infection.
toddlers are more prone to foreign body insertion into orifices as they try to explore those orifces. most common being nose and ear.
