A 6 year old Caucasian girl brought by her mother because she noticed that she has breast enlargement. Her height, weight, and developmental milestones were all within normal. No family history of related illnesses. No prior medical problems or treatments. Examination confirmed Tanner stage 3 breast development and axillary odor. Several dark pigmented spots seen on the back. CNS imaging was normal but skeletal surveys showed multiple cystic lesions in the upper and lower limbs. What is the best treatment for this condition?
A- Observation
B- GnRH analogues
C- Surgical removal of the adrenal gland
D- Exemestane
E- Hormonal replacement

The correct answer is D!

This is a case of McCune Albright syndrome (precocious puberty, cafe aut lait spots, and polyostotic fibrous dysplasia) and the treatment of choice is aromatase inhibitors such as Exemestane.

Do I qualify as a good question writer :cool: perhaps I should start my own qbank :p
