In any clinical scenario when all of sudden you have profuse vaginal bleeding after performing amniotomy and a fetal compromise shortly follows then you have to immediately think of Vasa Previa.
The boards loves to ask about Vase previa.

High Yield points include:

  • Risk factors: velamontous insertion of the umbilical cord, accessory placental lobe, and multiple gestation.
  • Vaginal bleeding is typical painless making some confusion with placenta previa.

Am I wrong but is vasa previa not a rupture of the umbilical vessels most times?

FHT should go tachy --> brady --> sinusoidal.
