Are these considered cytotoxic or noncytotoxic Type II HS rxns? Could someone review for me which ones are cytotoxic and noncytotoxic and why?

Pemphigus can be non-cytotoxic but not pemphigoid

Both pemiphgus vulgaris and bullous pemphigoid can be considered cytotoxic type II hypersensitivity reactions as in both conditions there can be activation of the complement which then results in the destruction of cells.

However, pemphigus vulgaris antibodies can alone can attach the keratinocytes desmosomes and results in acantholysis without the need for complement or inflammatory cells. Therefore, pemphigus vulgaris can be cytotoxic or non-cytotoxic type II HS reaction.

While in bullous pemphigoid the binding of antibodies at the basement membrane activates complement and inflammatory mediators. Activation of the complement system is thought to play a critical role in attracting inflammatory cells to the basement membrane. These inflammatory cells are postulated to release proteases, which degrade hemidesmosomal proteins and lead to blister formation. Therefore, bullous pemphigoid can only be of the cytotoxic type.
