A 33 years old HIV patient developed severe watery diarrhea resistant to all treatments. Stool acid fast stain showed elliptical eggs.
What is the most likely diagnosis?

A- Isospora belli
B- Cryptosporidium parvum
C- Cryptococcus
D- Giardia labmlia
E- Norwalk virus

The correct answer

It's funny how they play with us with words. Elliptical is a round shape with the stretched horizontal axis while oval is a round shape with elongated vertical axis. Isospora has elliptical shaped eggs. You can try TMP/SMX for treatment. Cryptosporidium has oval eggs. No treatment is successful. The correct answer is A

the answer is giardia~ what are you guys talking about

its isospora guys!! the presentation is very much a feature of isospora!! especially when he says elliptical cysts and treatment failure and acid fast .. treatment failure is typical of isosporaa and moreso in hiv patients
