Yes, we can. Most of the hospitals do not require Step 1. Athough there are few. Just make sure u apply 4-6 months in advance. And most of them require TOEFL.

It depends on the university you are applying to.
Most of them are willing to sponsor B1/B2 Visa which is a Visa that can be used to attend a business meeting or for a conference or for electives ..etc.

However, few universities have started to require J1 Visa even for the electives but that's still rare.

The only standard requirement for H1 Visa is to pass USMLE Step 3. Other than that it depends on the hospital that's willing to sponsor you.
Many US residency programs certainly happy if you did electives in US but it's usually not a requirement per se.

I find most of the electives offered charge fees of 1000-2000$ fee.
Is it the case with all the hospitals.
Can you please suggest me where can i get affordable electives

I know that St. Luke's and Cleveland Clinic both offer elective rotations for free. But am sure there are many more. Do your search in all universities.
