Here's how it looks like

As seen in the image the space where the pituitary should have been is filled completely by CSF.
The most common reason for this is herniation of the arachnoid through the diaphragm sella. Suprisingly, most patients do no have hypopituitarism and usually discovered accidentally.
Other rarer causes include;
Surgery, Radiation, Infarcted adenoma, Sheehan syndrome
