I have a question about hand washing.
What is the best time to do it, before the exam or before history taking!
Is it mandatory in all cases, like for example should I do it when a mother is talking about her child!
Should I put on gloves in any case!
Should I ask for SP permission to do it!

Hand washing before physical examination

You can wash your hands OR wear gloves, you don't need to do both of them. Perhaps, gloves are quicker to wear than washing hands and so it's preferred.
No, you don't need to do it if you are not going to touch the SP!
Best time, is of course right before you start the physical examination.
To familiarize yourself with the hand washing technique I suggest you have a look at csevideo as they have a video devoted for hand washing.
