Fellowship is training after main residency program:
So Cardiology is a fellowship in Internal Medicine.
Colorectal Surgery after General Surgery.
Spine Surgery after Orthopedics or Neurosurgery.....etc

You have to repeat the match process for fellowship the same process you did when you applied for Residency program.

Fellowship seats are limited...so you may noticed that why Residency in Univ program is better than Community Program...Since you have more researches in a Univ program & connections for fellowships.

So is there an exam or something ? i mean if i want to do the residency in my home country and apply for fellowship afterwords ? and does taking the elective course in the US help me for fellowship? as i heard that observership is not that helpful !
I may sound dumb but what does community program mean?!

I am speaking generally, not considering very rare occasions.
It is very difficult, if not impossible, to do U.S fellowship on foreign residency program, I know many cases who did U.S residency on their foreign Residency in order to get U.S fellowship.

Elective courses help in Residency program & connections :)
It would be good in CV, but I believe that Research is the key for desired fellowship.

Community program, is a program that is not directed or administrated by University. University based programs are better in researches & connections.

There is something called Community university affiliated program, is a mixed program that contains some faculty from the university which affiliated with & some non faculty who work at the hospital, this type is still lower than Univ program.

Univ program, is the best, no one can deny that, Academic & clinical learning, Researches, connections....
You know, not all Univ's ideal, some are very big names, some middle, some too low to the degree lower than Community program.

pls can you give me any info about work or practicing in USA after residency program?
does one have to do fellowship before you can practice in the state and what are the advantages of doing fellowship after residency??????
