It's very competitive

Ophthalmology residency is extremely competitive in USA. Even US grads from great universities such as Yale and Harvard are having trouble getting into it!

For an IMG I'd say here's what you need to do:

  1. Finish Ophthalmology in your country.
  2. Get 99 in step 1 and 99 in step 2 CK and pass the CS first time.
  3. Do hands-on Ophthalmology experience in USA (usually in the form of unpaid research volunteer).
  4. Get publications in and Ophthalmology research in highly recognized US medical journals.
  5. Make great connections with a specific residency program director and attendings.
Then after all that, I'd say you have a 10% chance

It's not that I am pessimistic or want to discourage you but this is the reality and I want to present it to it without false encouragements.

We have recently published a book entitled "How to Get that Training Post in Ophthalmology", which may be helpful for those wishing to take up ophthalmology as a career, particularly those based in the UK. It will give you an idea of the high standards that you need to aim for if you want to enter ophthalmology training in the UK. Please note that it is published for the UK in mind and is therefore not specific for the US; however, the general principles contained within should still be applicable.

For more details about the book, please visit: http://ophthalmologytraining.blogspot.com/

Good luck!
