Where does one start to complete the immunizations requirements for the USCEs? Is there any kind of a specific standard form I should fill? Do I have to just copy a lab report that I have the required antibody titers/took the shots and that would be enough? Since all that info would be in my mother language, do I have to translate it into English with a notary's approval or smth?

Depends on the place, each place has its own inmunization form....
