You don't need to mention the score points in details unless it's absolutely necessary such as a case of Alzheimer disease.
In other neurological cases, for example, epilepsy, you don't need to count the socre.
You can write in the notes something like the following:
pt oriented to T/P/P, momory/recall/concentration WNL.
