what is the most common cause of spontaneous bacterial peritonitis??
i think goljan mentioned it but i forget (and i think different sources say different bacteria), i know its asked alot on step1. thanks

Traditionally, three fourths of spontaneous bacterial peritonitis infections have been caused by aerobic gram-negative organisms (50% of these being Escherichia coli), with one fourth of these infections are due to aerobic gram-positive organisms (19% streptococcal species). E coli is demonstrated in the image below.

it is caused by both gm negative ( E.coli) & gm positive (strepto. pneumonei)
so always use 3rd generation cephalosporins to treat....

If it's a child with nephrotic syndrome, it's usually Strep pneumoniae. If it's an adult with cirrhosis, it's usually E. coli.
