You may a need a certificate from your dean

I assume that you gave your name differently to ECFMG because that's how it's written in your Passport, am I right?

In that case it's difficult to change your name in the passport. But what you can do, is to ask your medical school dean to write a letter certifying that you are the same person holding the different "middle names".

Generally speaking, the ECFMG tolerates differences in the middle name. Usually the middle name in US documents such as the driving license the the passport is represented by the middle "initial only" so your US name would be Moayed W Alizzi and that would match both names. However, I cannot guarantee for you that they'll accept it. The best course of action is to call them and see if that's OK with or you need that Dean's certificate that I told you about.
