Cholecystokinin

Your answer choice is A. Given that this patient is a Female, Fat, and in her Forties, her symptoms could be due to Gallstones (cholelithiasis).
After a fatty food ingestion, Cholescystokinin (CCK) acts by: increasing pancreatic secretion and gallbladder contraction, and decreasing gastric emptying.
In cholelithiasis, pain worsens after fatty food ingestion due to increase in CCK.
