The USMLE question will not tell you overtly that the patient is suffering from proximal or distal muscle weakness, instead, they'll give you clues "buzzwords" to make you think of these.

Examples of clues "buzzwords" that point to Proximal Muscle Weakness:
Inability to sit up or rise to standing, Inability to climb stairs, Inability to comb hair, Difficulty brushing teeth ...etc

Examples of clues "buzzwords" that point to Distal Muscle Weakness:
Inability to button up clothes, Inability to turn the door knob, Inability to write ...etc
