I am posting a series of private messages between me and Sabio and I thought it would be useful to share with you the conversation.

I am International medical student
I have the U.S citizenship also.
I am planning to do clinical elective next summer 2011.
I was so tired form searching every university in AAMC website for elective

I want good university or hospital but
not high fee

May u help me for that??

P.S:
I didn't take any Step, no need for visa bcz I hold U.S citizenship.

One university requested these:
Clinical experience
Evaluation in specialty of your elective
Proof Health and Accident Insurance.
What are these points especially first 2 points & how to get them??

Here in USA when students do their core clinical rotations they receive detailed evaluation performance from their supervisors.

Therefore, they expect you (the IMG) to have similar performance evaluations from your medical school and core clinical rotations. and this is what they are requesting here.

The second point is telling you that if you want to do Radiology elective then you don't need to submit prior evaluation for that matter.

There's no difference they are just stressing the point that you evaluation should be written and signed by the professors whom you have worked under during your clinical rotation in your medical school curriculum.

By the way,
Getting an elective while you are still a medical student is much easier than when you graduate.

So I urge you do it now before it's too late and it will be very difficult for you get one.

Please note the following electives are not being offered to foreign students at Mount Sinai School of Medicine. If available at our affiliates you may apply: Anesthesiology, Sub-Internship in Medicine, Emergency Medicine, Pulmonary, Palliative Care Consultation, Introduction to Internship and the Critical Ill, Introduction to Emergency Ultrasound, Clinical Experience in Oriental Medicine, Palliative Care Consultation.

What does it mean "Sub-Inernship in Medicine"???
also help me in digestion of whole sentence.

They are basically telling you that they don't provide elective rotations except if it is available in their affiliated hospitals.
The term sub-internship means that you do an internship in a hospital which is not affiliated with your medical school. Basically it just means internship in your case.

Which is better to do 2 electives;one in Surgey,the other in medicine.
Each elective one month duration or doing one elective 2 months duration?
especially if student does not decided particular specialty(medicine or surgery).

Thanx in advance.
Focusing on a single specialty right from the start will greatly enhance your chances.
So I suggest you do a two months medicine or two months surgery better than one month each.

So How to do with that??

Another Question, I noticed that many universities have affiliation with what is called VA hospitals, What are these? Are they considered better that University hospitals?
VA Hospitals means medical facilities that are operated by the department of Veteran Administration. These hospital provide services to veterans (military retirees). Am not sure if such hospital actually provide elective services if you are not army affiliated.

1) If such a IM program requests 4 months USCE, and I did 2 months IM & 2 months Surgery Electives, Am I eligible for that program?
(Assume that is high ranked program, Must USCE be in the same program specialty?)

2) Are Neurosurgery, orthopedics & ophthalmology...electives help in General surgery residency ? (Although Neurosurgery & orthopedics are have their residency programs independent on General surgery residency)

i.e I participate in Surgical & Internal Medicine Rounds....Can I say that I did 2 months surgery & 2 months medicine electives? suppose I got LORS from both of them.

Yes you can.
But make sure that they mention the details in the LoR.
Like the internist saying he was staying with me for these days and he was with the surgeon for these days ...etc
Again. Such an elective is not as good as an elective that is specialty focused.

I will try to have independent LORs, in other words, nothing said in LORs that I did them at the same time.

I want to do this to help in both medicine & surgery residency.
For Surgical programs, I will add LOR of the surgeon
for IM programs, I will add LOR of the Internist

(So it will appear for IM programs I did 2 months, & for Surgery programs that I did 2 months too.)

So can I do that ?

I meant that I will do them at the same time. In other words, after finishing the surgery rotation & observe some surgries, go with the internist & participate with his activities either if he has consultations,research or out patients.
I do not think that the surgeon will not give me breaks in order to that, do you ?
Or even I don't mind to stay to evenings & participate with the internist if he has evening rounds or research.

I know you may tell me that the doctor will not write that I stayed with him all the time & I went to other electives. But suppose that I know the doctor & write a good lor without any info to the other elective.

Then when the match start, I will upload the surgery lor for surgery programs & the same story with IM programs.

So Can I do what I think? or they will punish me because I can not put the same dates in CAF ?

May I upload LOR without write about it in CAF?

or forget the 2nd elective, make a researches, so I can benefit from Surgery rotations & clinical researches with the internist?

I believe you can't do it even if the attending is close to you. Because you have to specify dates of start and finish. If you put both the IM and surgery with the same start and finish date then that would be awkward.
In CAF you have to mention your USCE and you don't want them to miss that. There's no point in having an LoR for an experience that's not mentioned in the CAF.
