I noticed that most of the cases in the CS cases are having a second minor differential fabricated.
For example a patient with depression symptoms but at the same time has for example cold intolerance to make us think of hypothyroidism.
My question is;
Should we get this second differential in the exam and if not are we going to the fail the case?

Yes, it's important to get that second and third differentials when there are clues in the history and PE.
But I don't think they'll fail in the case if you missed that. The first differential is absolutely needed but more correct differentials will add to your score and missing them does not necessarily constitute a failure.
