My family name is TAN, given name is KIAN BENG. The name appears on my scheduling permit is KIAN BENG TAN.

However, on all of my identification forms, my name appears as TAN KIAN BENG (Malaysian Chinese have their family name in front of the given name). If I were to change the sequence of names by submitting Form 182, I will have to switch my family name in place of my given name and vise versa, meaning that I have to state KIAN BENG as my family name and TAN as the given name, but this is "not correct". And this incorrect sequence of names will be on my ECFMG record forever.

The name on my ECFMG record is absolutely correct. It is just that the sequence of names on my forms of identification does not match the one on the scheduling permit.

Please guide me on what should I do.

What is exactly written in your identification cards and documents?
Tan Kian Beng
or
Tan, Kian Beng
does it mention what's the first and last name?

Thanks for your reply Sabio!

The name on all of my forms of identification is "Tan Kian Beng" (without the comma)

ECFMG has replied to me saying that I have to change the sequence of my names by submitting form 182.

The problem is that once I change the sequence then I will have a "Wrong Name" on ECFMG record in the future.

What is on the ECFMG record is absolutely correct, just that the sequence of names on my identification forms differs.

Any other suggestions mate?

You either change your IDs or change your ECFMG record, they have to match exactly.

See which one is easier to change. I would guess changing ECFMG records is easier.

Thanks step taker!

I just found out that the name on my US Visa (B1/B2) is stated as
Surname: Tan
Given Name: Kian Beng

But unfortunately it does not have my signature on the same page. This visa is still valid upto year 2020 but it is on my "old passport".

Can I use this US Visa as the form of identification supplemented with my current passport (which has my signature)? Of course the sequence of names on the current passport is Last name First name (Tan Kian Beng).

I have also called to Prometric in Kuala Lumpur, Malaysia where I will be taking step 1. They told me that they understand this problem (since they are Malaysians) so I can just present my passport and I will be admitted for the test.

However, Step 2 CS can only be given in the States and I may face the same problem again in the future.

Even if you get away with it for the Step 1 exam, you will still be facing problems in the future.

If you are planning for a career in the US, the ECFMG record will be with you for the rest of your life.

I suggest you fix this issue now before it gets more complicated in the future.

Many other Malaysians have taken the test with their chinese name.

Just maintain as it is, Tan = surname, Kian Beng = name. If the orientation becomes Kian Beng Tan, but your credit card reads Tan Kian Beng, it is OKAY. Please call up the Malaysian prometric center to confirm, because the ECFMG in the US has no clue actually.

I have a main problem my given name in passport is natasha singh and there is nothing written in my surname in d passport so what shall i write in rest name field and last name field for my ecfmg form...i have to apply for my ecfmg id.........please help me out for this

I had a similar problem..
According to my passport, I have a first name and a middle name,but no last name/surname.

I called up ECFMG, they told me to fill my entire name i.e (first name+middle name as in passport) in the last name field, they asked me to leave the First name field as blank. Thereafter, I received an email from ECFMG to fax a copy of passport, they verified within two days.

If my answer was not helpful,I suggest you call ECFMG, they are very helpful.

This is the email I got,
"Dear Doctor:

We have received your request for a USMLE/ECFMG identification Number. We are unable to process your request because it is incomplete. You have not indicated a first name. In order to obtain a USMLE/ECFMG identification Number, please resubmit your request. Be sure to include your full name (given name, surname and any middle names.)

If you only have one name, you must fax a photocopy of your current passport showing this one name to Attn: ECFMG/CSeaner at (215) 386.9767. Once we have verified that you only bear one official name, we will issue your identification number. ECFMG must receive this fax within five business days. If we do not receive this documentation, your request will be deleted and you will be required to re-apply if you wish to be issued an identification number.

If you have any questions, please contact Applicant Information Services at (215) 386.5900 or [email protected].

I had a similar problem..

does that count as 'having applied to ecfmg before' ?

Since you already have USMLE id number, I think you have to have to submit form 182 along with the appropriate documents to ECFMG to change your name.

:) thank u so much for d reply........i just wanted to ask so i should fax them a copy of my passport when i apply for d ecfmg i.d or only if they ask for it............and that also means that they didnt have a problem that if we dont have a surname in our passsport

1) First, fill out the application i.e with your entire name in the last field column.
2) You'll receive a mail similar to the one I posted.( Check spam folders too.)
3) Wait for the email and only then fax your Passport.

They have no problems if we have no surname. :)

I just wanted to ask that did u put a comma or n/a in your rest name field as my some friends told me to do so....or you just left it blank?

Your welcome!
I did not put any comma in the rest of the name column. I left it blank.
My advice- Try to stick to your official name. Don't put something you don't have documentation for.
