So I've seen a question saying that UTI treatment should continue for 7 days in the pregnant woman rather than the regular 3 days regimen.

Would please tell me if there are other situations when antibiotic treatment should continue for more than three days in a case of simple lower UTI.

When to give 7 days antibiotic therapy in cystitis

Antibiotic treatment for simple cystitis is TMP/SMX for three days if the geographic area has less than 20% ecoli resistance otherwise you give one of the quinolones such as ciprofloxacin or levofloxacin.

However in complicated cystitis such as those with stones, strictures, tumors, anatomic abnormalities, or obstruction then you give it for 7 days.

Also it's recommended that you give 7 days treatment in the following situations:

  • Male patients (don't forget to to do Ultrasound first)
  • Pregnant patients (don't give quinolones)
  • Previous recent UTI
  • Previous recent antibiotic use
  • Age above 65
