I took my step 1 after doing Kaplan + UWORLD + First Aid. standard stuff.

I was in for a nasty surprise with the audio- visual clips. I had completed ALL the questions in uw and 2 exam forms, and there was not a single clip.

the clips are not difficult, its just that anything unexpected during the actual exam is unsettling.

most of them were heart murmurs :ASD, VSD, PDA etc. did not get any valvular lesions. so make sure you practice those.

with prep, you can get all of them right!

Yes, you are right that UWorld has little media questions
But am now doing Kaplan and I can see several video and audio clips
Did you see those when did Kaplan?

Another question please what was your score and those media clips were they having enough info in the question stem to help us answer?

Please help, you made me worried

i had limited time to study. so I used the Kaplan books throughly, but did not use the cds.

Strange thing is, that no one I knew who had taken step 1 even mentioned the media clips!:) guess I should have joined usmle-forums before step 1....regret that now...

UW has all the histo slides, n x-rays, MRIs n CTs. so I figured thats all there is.

See, there's no need to get worried. those clips, as I mentioned earlier, were not difficult per se,most were heart murmurs, along with a clinical case. U know them or you dont know them. its just unnerving to have something new spring up on you during the exam. you are already aware of this, so you can avoid being caught off-guard. I just want to emphasize that you cannot depend on UW to prepare you for this.

my score is expected shortly.

Can you get the question right using only the clinical case? Or is the knowledge and experience of hearing the sounds essential to get the question right?

a few questions were a proper clinical case with the media. those can be solved using only the text. but i remember a few questions which were like:"2 year old child brought by parents for episodes of turning blue after crying. your auscultation yields following sounds." followed by the clip. so obviously u need to hear the murmur before u can conclude.

my advice is, dont go without practicing the media. its easy to think u can manage with the clinical case alone, but thts not always possible. and even it is, its always better to confirm your diagnosis with the murmur.

i wouldn't be too stressed about practicing media clips etc...as far as Step 1 or Step 2CK goes...

from my experience as well as others...these "media" questions don't amount to anything more than a total of 2-3 questions out of the WHOLE exam....

yes i said it...WHOLE exam...not per block! ;) of course its good to get points for any question...especially if it's easy, but the video clips are very straight forward.

murmurs are popular as well as videos of watching a kid play and determining milestones and such. the clips on the USMLE cd or practice questions are pretty accurate. personally i think that is more than enough.

um..I didnt get any visual clips. but the tutorial demonstrated a video of an old man with bradykinesia and tremor, so its possible to get something like that.

Another question though. What did you mean by visual clips? Is it like a video clip of tremor and such?
Sorry for that. Yes....it could be a clip of a little child just playing or hopping...and u are supposed to note the milestones. Or as the previous post mentioned, an eldery person with slow movements etc...

Or you may get a computer generated one....u know...animated type but those are usually so you can listen to heart sounds. :rolleyes:

anyone who is preparing for USMLE

Anyone preparing for this exam should expect audio visual clips.
there shouldn't be anything unsettling about that at all, they should be happy there is something to help them during the exam.
