What if your H1b expires during residency

I'm working in the US with an H1b now and trying to prepare for the USMLE exam. With a full-time job is hard, and I'm giving myself 2-3 years to do it. Since the H1b can only be valid for 6 years, it's possible that even if I get into a residency program that agrees to apply H1b for me, it'll expire in the middle of my residency. Is there any solution to this situation?
Any information will be appreciated.
