ESR increases in cases of inflammation due to the secretion of IL-6 by macrophages during inflammation which acts on the liver to increase the production of acute phase reactants like haptoglobin, ceruloplasmin, crp etc and fibrinogen levels also goes up which causes increased rouleaux formation thus increased ESR
