Default Med School Verification form- Mayo elective

Hi, everybody!

I am a final year international medical student and I had all my documents ready to upload to Mayo clinic's online elective application form, when I found out that the letter of recommendation must be uploaded by the recommending professor/provider and the dean/designated appointee must complete and fill the Medical School Verification form. While asking my LoR writers to upload the LoR has been a comparatively easy task, getting my dean or his PA to upload the Verification form has been difficult because it is the busiest time of the year in my Medical college with all the students giving their final exams. When I emailed the elective coordinator regarding my issue he/she said it is fine for any 'staff member' to upload. Here on this forum, I want to ask anyone who has already applied to Mayo earlier or this year to help me with this issue and guide me regarding whom else I could approach to upload the verification form on my behalf.

I asked my Dean to fill out the form for me. I'm not sure if any other person is allowed to do it. Sorry

I just asked one of the professors who wrote my LoR to fill it out for me. Works just fine!
