Skin lesion from Iraq duty!
A US soldier returned from Iraq with a skin lesion on his right upper arm. The lesion was papular then developed into a nodular 4X5 cm crusting with irregular sharp border. Giemsa stain of biopsy showed inclusions with histiocytes. Which is the most likely infecting organism?

A- Leishmania major
B- Strongyloides stercoralis
C- Histoplasma capsulatum
D- Plasmodium vivax
E- Toxoplasma gondii

I agree with you.
Two bugs are commonly seen in soldiers deployed to Iraq. Leishmania and Strongyloides.
The case is describing cutaneous leishmania. The inclusions in histiocytes are Donovan bodies.

Agreed - none of the others fit. Leishmania it is.
