A 35 year old man complains of tightening chest pain for 6 hours radiating to the neck. He also has shortness of breath and lightheadedness. He has a family history of ischemic heart disease. He inhaled cocaine an hour before the onset of pain. Cardiac biomarkers are normal except for extremely high creatine kinase. What is the most likely diagnosis?
