Default Carcinoid syndrome

So, we've learned that carcinoid tumor only causes carcinoid syndrome once we have a liver met. Because otherwise, the liver metabolizes the serotonin.

Further, we've learned that carcinoid tumor can cause RIGHT HEART damage, but not LEFT HEART damage, because the lungs metabolize serotonin.

So, my question:
How does carcinoid tumor EVER cause systemic symptoms? It sounds like the serotonin is metabolized either in the liver or in the lungs. So how would it EVER reach systemic circulation?
