I want your advice,
I have got accepted for an observer ship for 4 weeks at Cleveland Clinics Critical care/ICU
but I am confused which is better for my CV an observer ship there or doing an elective for 4 weeks in less important hospitals.

clerkship is anytime better than observership.....but then it depends on your graduation status....
if u r already a graduate then u wont be able to do clerkships and so observership is ur only chance

I am still in my last year in my medical school.

then you suggest to take the elective not observer ship even though Cleveland clinic is Number 1 in the state in cardiology.

knowing that I want to match in cardiology later on

hmm has cleveland offered electives too??? if yes then go for it..
if it is only observership then its your choice whether or not to take it.....try applying for more clerkships meanwhile and keep this as an open option.....but many universities offer clerkship only if you have a step 1 score and there are a very few universities which do not require it....so balance your options and chose the best one...if you dont have any offers till then, then doing the observership wont be any harm...but yea choose for yourself
