Hello everyone , i just did a q in Uworld about pulsus paradoxus and it said you find the difference by deflating the cuff until you hear the sounds in both expiration/inspiration... Can someone elaborate on that ?? e.g how exactly is it done , what are these korotkoff sounds etc.. The only thing I know about them is that you measure BP with the 1st and the Last korotkoff...

Here its only about the first Korotkoff sound.
So you increase the pressure to above SBP and then reduce it slowly. So say you increased to 150 and the actual pressure is 130. When you reach 130, you will hear Korotkoff sounds only in expiration. So there will be the sounds, then silence, then the sounds again. Keep reducing the pressure by 2/second while checking for disappearance of this gap in between the Korotkoff sounds. So now there is a sound for every beat. It should happen by the time you reach 120 in this case. Else its pulsus paradoxus.
