basically, if there is B12 deficiency without B9 deficiency, then giving B9 to the patient is going to "burn" or steal the little B12 left in the body, so B12 deficiency manifestations would increase

megaloblstic anemia can be due to either folate deficiency or B 12 deficiency or both....if it is due to both folate and b 12 deficiency and you give only FOLATE supplementatin....thymidine synthesis will resume with supplemented folate and with whatever little vitamin b 12 is there in body....because of this...there will be acute shortage of vit b 12 for conversion of methylmalonyl to succinyl co A (essential for myelin synthesis)....leading to abnormal myelin synthesis or worsening of neurological features caused due to vit b 12 deficiency.
