But why its not multi infarct dementia ,the patient has hypertension .Moreover ,it seems step like progression .
Everytime I answer multi infarct ,it turns out to be Alzheimer's disease

A. Alzheimer's disease since it's been there for many years and only recently become worse.
There is lack of information in the question stem to point to any other pathology.

point is if pt had multi infarct pt would have focal cns signs like weakness in left arm or right leg or sensory loss in a limb but pt has no such focal defects thus its not multi infarct .

lol, just in case you want to read a specific dementia or to compare between them, i didnt read the whole article, but read the dementias in which i struggle a lot, including pick vs lewy body and multi-infarct dementia
