DUB

• premenarchal -> could be cancer, precocious puberty, foreign object
• post-menopausal -> carcinoma until proven otherwise
• reproductive age -> most common is anovulatory cycles

Most common DUB is in reproductive age, constant high levels of estrogen leading to anovulatory cycles: DOC is progestins (usually medroxyprogesterone acetate), second choice OCP's. Endometrial ablation or hysterectomy only if patient has failed medical treatment and has no interest in childbearing.

 

after failing ocp can try leuprolide...if that too fails..then its ablation.

 

btw for unstable DUB, rx is estrogen !!

one more thing...clue to dub bleeding in q will be CRAMPLESS bleeding with CLEAR cervical mucus [ that means it is E primed ]///

Right - for sudden heavy bleeding, high-dose estrogen is given to induce rapid growth of the lining and stop the bleeding. When the bleeding is controlled (<24 hrs one hopes), progesterone is added or the patient is tapered to OCP.

DOC is drug of choice, doesn't have to do anything with oral contraceptives (OCPs).
