A 23-year-old female presents to the emergency room complaining of pelvic pain and some uterine bleeding. She does not recall when she had her last period. An ultrasound is ordered and she is diagnosed with a hematosalpinx. What is the most common cause of a hematosalpinx?
A. Cryptomenorrhea
B. Transverse vaginal septum
C. Tubal carcinoma
D. Ectopic pregnancy

The correct answer is D.
Ectopic pregnancy is the most common cause of a hematosalpinx (bleeding into the fallopian tubes). Endometriosis and tubal carcinoma are less common causes of hematosalpinx. Another less common cause of hematosalpinx is a transverse vaginal septum, which obstructs menstrual blood flow (cryptomenorrhea) and backs up in the fallopian tubes.
