For eg, a patient with acute appendicitis comes and demands painkillers in the beginning, ofcourse we're gonna say no; but at the end of the history and p.e., when we tell him about the tests we have to do and whatnot, and he asks for painkillers again; do we say 'Sorry sir we have to wait for the results of the test before I can give you any medications' or 'Sir I will give you some painkillers now to relieve your pain, but I will not be able to give you definitive treatment until I get the results of the test back'.

The second one seems far more solid to me.

Whenever the SP asks for pain killers whether it's in the beginning or at the end of the encounter you should say
"Yes. Definitely I will give you pain killers and I will do everything I can to ease you pain but please let me finish the history and physical exam so that I know the exact problem so that I can give you a definitive treatment as well"
Before you leave, tell him "I'll go order the nurse to get you the pain killer while we prepare for further testing".

The idea here is not "medical", do not think whether the pain killer is indicated or not, just think that the SP is trying to see how sympathetic and passionate you are.

As Steptaker puts it , the SP is basically trying to gauge how you are going to respond.
Never answer in the negative "I am not going to give you pain killers now"
always give a positive response and add to that response.
I couldn't have put it in any better words than what steptaker has mentioned

Another way of answering the same would be" I understand that you are in pain, and i will do everything i can to get to the bottom of it and give you the meds at the earliest"

A Kaplan SP was very happy when i told him"..... once i get to the cause of the issue ,my first agenda is to relieve u of that pain, so pls bear with me."
